Oregon Health & Science University is seeking a licensed pharmacist to provide comprehensive pharmaceutical care services for oncology patients ranging from neonates to elderly patients. This position operates within a flexible, coordinated care model that integrates patient care transitions with traditional centralized pharmacy functions.
As an Oncology Pharmacist, you will function at higher decision-making levels regarding medication therapy, proactively affecting medication use by making recommendations during prescribing decisions or making independent therapy decisions in cooperation with healthcare team members. You'll facilitate medication reconciliation, provide discharge counseling, and coordinate medication delivery from admission to discharge while working collaboratively with interdisciplinary teams including providers, nurses, case workers, and pharmacy staff.
Key Responsibilities:
- Identify, resolve and document complex therapeutic problems with advanced knowledge application to oncology patient populations
- Interpret and evaluate prescriber orders for accuracy, completeness and appropriateness
- Monitor therapeutic response and adverse effects of medications to guide drug therapy decisions
- Provide drug information and education to prescribers, nurses, patients and families
- Assist in developing treatment guidelines for patients with specific oncology diagnoses
- Supervise pharmacy technicians, students, interns and residents
- Participate in education programs and peer development activities
Experience Requirements
Inpatient hospital pharmacy practice experience including oncology, acute care, and specialized patient populations
Education Requirements
BS in Pharmacy or Pharm.D. plus Practice Residency/Equivalent Experience or Specialist Certification (BCPS, BCOPS, etc). Current Oregon State Board of Pharmacy license required.
