Clinical Pharmacist

As a Clinical Pharmacist, you will interpret medication orders to ensure safety and proper use, coordinate medication-related problem solving between pharmacy, nursing, and prescribers, and monitor medication therapy for significant interactions. You will act as a consultant in pharmacokinetics and adverse reactions to healthcare professionals, participate in therapeutic drug monitoring programs, and maintain comprehensive pharmacy patient profiles.

Key Responsibilities:

• Interpret medication orders and monitor for compatibilities, allergies, and potential administration errors
• Coordinate medication-related problem solving between departments with sound clinical judgment
• Monitor and intervene in cases of significant drug interactions and therapeutic duplications
• Act as pharmacokinetics consultant to physicians, nurses, and healthcare professionals
• Participate in therapeutic drug monitoring and intervention programs
• Compound and dispense pharmaceuticals including sterile products and chemotherapy
• Provide medication education to patients, families, and healthcare staff
• Maintain hospital formulary and ensure compliance with medication policies
• Document clinical activities and participate in quality improvement initiatives
