Clinical Pharmacist PNE

As a Clinical Pharmacist PNE, you will play a crucial role in ensuring medication safety and optimizing therapeutic outcomes for patients. You'll interpret medication orders, monitor drug therapy, and serve as a clinical consultant to physicians, nurses, and other healthcare professionals. This role involves comprehensive medication management, from processing orders in the Electronic Health Record to compounding sterile products and providing patient education.

Key Responsibilities:

• Interpret medication orders and monitor for drug interactions, allergies, and potential administration errors
• Coordinate medication-related problem solving between pharmacy, nursing, and prescribers
• Evaluate medication appropriateness and communicate recommendations to healthcare teams
• Participate in therapeutic drug monitoring programs and adjust therapy based on patient condition
• Compound and dispense pharmaceuticals including sterile products, chemotherapy, and parenteral nutrition
• Provide medication education to patients, families, and healthcare staff
• Maintain comprehensive pharmacy patient profiles and documentation
• Respond to emergency situations and assist with medication management during cardiac arrests
