Oregon Health & Science University is seeking a licensed Pharmacist to join our Oncology team and provide specialized pharmaceutical care for patients ranging from neonates to elderly adults. This position offers the opportunity to work in a leading academic medical center while making a direct impact on patient outcomes through advanced medication therapy management.
As an Oncology Pharmacist, you will function at higher decision-making levels regarding medication therapy, proactively affecting medication use by making recommendations during prescribing decisions or making independent decisions about medication therapy in cooperation with healthcare team members. You'll facilitate medication reconciliation, discharge counseling, and coordinate care from admission to discharge while working collaboratively with interdisciplinary teams including providers, nurses, case workers, and other pharmacy staff.
Key Responsibilities:
- Identify, resolve and document complex therapeutic problems using advanced knowledge for oncology patient population
- Monitor therapeutic response and adverse effects of medications to guide drug therapy decisions
- Respond to drug information requests from physicians, nurses, patients and families
- Direct and evaluate the performance of students and residents in clinical settings
- Develop treatment guidelines for patients with specific oncology diagnoses
- Participate in interdepartmental education programs and peer development
- Supervise pharmacy technicians and verify accuracy of medications prepared and dispensed
Experience Requirements
Inpatient hospital pharmacy practice with oncology experience preferred
Education Requirements
BS in Pharmacy or Pharm.D. PLUS PGY-1 Practice Residency or Specialist Certification (BCCCP, BCPS, etc)
