Pharmacist

Independent Health is seeking a licensed Pharmacist to join our Reliance Rx specialty pharmacy team. This role offers the opportunity to provide comprehensive pharmaceutical care while working with specialty medications and sterile product preparation in a dynamic healthcare environment.

As a Pharmacist, you will be responsible for preparing and dispensing medications including sterile admixtures, evaluating physician orders for appropriateness of therapy, and maintaining optimal patient-specific therapy through clinical management. You'll work closely with physicians, nurses, and patients to provide drug information, clinical consultation, and patient counseling.

Key Responsibilities:

• Prepare and dispense medications and sterile admixtures in accordance with physician orders and regulatory standards
• Evaluate medications for appropriateness of therapy and monitor patient outcomes
• Maintain patient medication profiles and conduct clinical monitoring including drug interactions and laboratory monitoring
• Provide drug information, clinical consultation, and patient counseling to healthcare teams
• Coordinate pharmaceutical services and participate in care planning and discharge planning
• Ensure compliance with URAC, ACHC standards, and federal/state regulations
• Supervise medication preparation and maintain professional relationships with pharmacy staff
