Clinical Pharmacist Specialist

Jefferson Health is seeking an experienced Clinical Pharmacist Specialist to join our pharmacy team at the Center City location. This role offers the opportunity to make a direct impact on patient care by overseeing medication distribution systems and ensuring optimal drug therapy outcomes for patients across our healthcare system.

As a Clinical Pharmacist Specialist, you will be responsible for identifying, preventing, and resolving drug therapy problems while ensuring each medication is indicated, effective, safe, and convenient for patients. You'll oversee the distribution of medications throughout the hospital, implement quality pharmacy service processes, and serve as a mentor to pharmacy students, residents, interns, and technicians.

Key Responsibilities:

• Oversee medication distribution systems and ensure compliance with hospital policies and formulary requirements
• Apply pharmacokinetic principles to complex patient problems and provide evidence-based recommendations
• Identify and document medication errors, adverse reactions, and near misses in electronic reporting systems
• Train and precept pharmacy students, residents, interns, and technicians
• Collaborate with healthcare teams to optimize patient medication therapy
• Promote compliance with P&T Committee policies and procedures
• Provide quality patient care demonstrating high-level expertise and clinical competency
