Under the supervision of the Director of Pharmacy and clinical coordinator, you will coordinate pharmacy department activities to provide the highest quality of pharmaceutical care to all patients served by the hospital, while ensuring patient safety. You will conduct 50% or more of professional time in direct patient care activities including interdisciplinary rounding, patient counseling, and participation in patient care committees.

Key Responsibilities:

  • Provide direct patient care including observation and evaluation of patients and their medication-related needs
  • Initiate, modify, or discontinue patient-specific pharmacotherapy
  • Conduct ongoing pharmacotherapeutic monitoring and follow-up in collaboration with other health professionals
  • Participate in interdisciplinary rounds and patient care committees
  • Provide pharmaceutical care across multiple service areas including General Medicine/Surgical, Intensive Care, Emergency Care, Oncology, and Outpatient Services
  • Apply antibiotic stewardship and infectious disease expertise

Experience Requirements

Completion of postgraduate clinical training (PGY-1) or 3+ years of qualifying experience required, previous Antibiotic Stewardship or Infectious Disease experience required

Education Requirements

Current state Registered Pharmacist License required, B.S. degree in Pharmacy required, Doctor of Pharmacy degree preferred
