As a Clinical Pharmacist, you'll work side by side with fellow healthcare providers in an interdisciplinary team approach to deliver patient-and-family centered medication management.
The Clinical Pharmacist practices as a pharmacy generalist in one or more patient care areas, ensuring appropriate therapy and institutional outcomes. You'll consistently provide patient-centered pharmaceutical care by proactively reviewing and processing orders, preventing and resolving drug-related problems, and performing daily in-depth patient profile reviews to assess, monitor and recommend appropriate drug therapy.
Key Responsibilities:
- Evaluate medical literature to optimize evidence-based care and monitor drug therapy regimens
- Document clinical activities, adverse drug events, and interventions according to institutional protocols
- Engage with clinical pharmacists and leadership for complex clinical situations
- Provide patient education on drug therapy, adverse effects, and proper medication administration
- Participate in medication use evaluations and quality improvement projects
- Serve as preceptor for pharmacy students and provide educational in-services
- Support cost-savings initiatives and medication stewardship programs
Experience Requirements
1+ years relevant pharmacy experience, ASHP-Accredited Post Graduate Year 1 pharmacy residency or Pharmacy Specialty Board certification preferred
Education Requirements
Doctor of Pharmacy degree, active Georgia Pharmacist license required
